A very simple filter that #Nostr clients can implement to neuter dangerous content (CP, porn, gore) in reply #spam would be disabling autoloading of pictures and videos (including thumbnails) for non followers.
Eventually that could be expanded to allow auto rendering of content to those within a web of trust.
This would prevent other, non trusted individuals, from forcing your device to render, load, download, or display dangerous media.
